Patients are seen by appointment only. I make every effort to minimize the wait time, and available for urgent consultations when referred by a physician.

Office Hours

• Monday 11:00am-5:00pm
• Wednesday: 10:00am-6:00pm
• Friday: 10:00am-6:00pm
• Saturday: by appointment only, call 718-888-0980

Appointments are made by calling at 718-888-0980. Cancellation of appointments is preferably 24 hours prior to the time of the original appointment. This helps me to provide timely and efficient medical services. Saturday a.m. office hours are available on a limited basis.

New patients

New patients should plan to arrive at the office 10 to 15 minutes prior to the time of their appointment. There are a number of forms, including prior history, review of systems, and privacy statements that will need to be completed before seeing the doctor. Please be prepared with the following documents:

• Insurance Card
• Medication List
• Physician list with addresses to whom communications should be sent
